Good Faith Estimate (No Surprises Act)

At Horizon Counseling Services, we are committed to transparency in the cost of care. Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” (GFE) explaining the expected cost of your medical and mental health services.

This information is provided in compliance with federal law.

What Is a Good Faith Estimate?

A Good Faith Estimate is a document that outlines the anticipated costs of counseling services for individuals who are not using insurance or who choose to pay out-of-pocket.

This estimate helps you understand your expected financial responsibility before beginning services.

Who Is Eligible to Receive a Good Faith Estimate?

You are entitled to receive a Good Faith Estimate if you:

  • Do not have insurance
  • Choose not to use your insurance
  • Are paying for services out-of-pocket

What Does the Estimate Include?

Your Good Faith Estimate will include:

  • The cost per session
  • The estimated number of sessions
  • The total expected cost of services
  • Any additional fees, if applicable

Please note that the estimate is based on information known at the time and may change depending on your needs or treatment plan.

When Will I Receive My Estimate?

You have the right to receive a Good Faith Estimate:

  • At least 1 business day before your scheduled service (if booked at least 3 days in advance)
  • At least 3 business days before your service (if scheduled further in advance)
  • Within 3 business days of requesting an estimate

You may also request a Good Faith Estimate at any time before starting services.

What If My Bill Is Higher Than Expected?

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the charges.

You can begin a dispute resolution process through the U.S. Department of Health and Human Services (HHS).
